Legendary Resistance (3/Day).If The First Sigher fails a saving throw, it can choose to succeed instead.
Aura of the Sighing Sickness.At the start of each of The First Sigher's turns, each creature within 30 feet of it must succeed on a DC 22 Constitution saving throw or take 21 (6d6) necrotic damage and be infected with the Sighing Sickness. While infected, a creature's hit point maximum is reduced by 10 (3d6) every 24 hours. The reduction lasts until the sickness is cured by a 'Greater Restoration' spell or similar magic.
Gaze of Despair.Any creature that starts its turn within 60 feet of The First Sigher and can see it must succeed on a DC 21 Wisdom saving throw or be frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Actions

Multiattack.The First Sigher makes three attacks: two with its Void-Touched Grasp and one with its Sorrowful Wail.
Void-Touched Grasp.Melee Spell Attack: +13 to hit, reach 10 ft., one target. Hit: 24 (4d8 + 6) necrotic damage plus 18 (4d8) force damage. The target must succeed on a DC 22 Constitution saving throw or its hit point maximum is reduced by an amount equal to the necrotic damage taken.
Sorrowful Wail.The First Sigher releases a haunting wail. Each creature within 60 feet of it must succeed on a DC 21 Wisdom saving throw or take 36 (8d8) psychic damage and be stunned until the end of its next turn.
The Last Sigh (Recharge 5-6).The First Sigher exhales a massive cloud of crystalline void-spores in a 60-foot cone. Each creature in that area must make a DC 22 Constitution saving throw, taking 77 (22d6) necrotic damage on a failed save, or half as much on a successful one. Creatures that fail the save are also incapacitated by profound sorrow for 1 minute. An incapacitated creature can repeat the saving throw at the end of each of its turns.

Once a high-ranking priest of Saint Elara, he was the first to succumb to the "Aether-Rot" after a Void-Shard experiment went horribly wrong in the capital. Now, he is no longer a mere husk; he has become an Avatar of the Void, his body a translucent vessel for entropy, pulsing with a sickly purple light that warps the very fabric of reality.

His mournful sigh is no longer a sound, but a metaphysical pressure that crushes the hope of those nearby. He is a living conduit for Ythul, the Forbidden Deity of the Void, and his every step blights the earth, turning vibrant life into crystalline dust.

Secret Motivation: The First Sigher’s original consciousness is trapped in a state of eternal, screaming witness. He is being used by Ythul to weave a "Web of Sorrow" across Aethervale. If he can infect enough high-level casters, their combined magical resonance will tear a permanent rift to the Void in the heart of Wyn_Dor.

Adventure Hook: The Arch-Wardens have realized that the First Sigher is no longer a victim to be saved, but a catastrophe to be contained. They have issued a "Codex Zero" order: the Sigher must be destroyed with a weapon forged from "Pure Aether," or the Sighing Sickness will become incurable as it evolves into its final, world-ending form.
